Health Plans
There are many types of Health Plans that New Ventures has developed for healthcare organizations, health councils, counties and communities. Health Plans are shaped by a Community Needs Assessment (CNA), Community Needs & Assets Assessment (CNAA) or Community Health Needs Assessment (CHNA). All are very closely related, and Anne’s focus is on community needs, services, gaps and the assets that communities can leverage to address these needs. Her plans are shaped by working collaboratively with a community “Core Team” and by involving communities in shaping these plans – through listening sessions, key informant interviews, surveys, focus groups and town halls.
“It’s important that plans be shaped by our community voices. As the consultant, I’m a vehicle to bring forward the voices of the community, and guide and leverage the community’s expertise in a plan that is well-informed. The plan serves as a practical roadmap.”
